FollowUpThen makes it easy to remember follow ups, just by including them in your email's recipients. It's free to sign up and then you just include when to follow up in the email address (ex. nextweds@followupthen.com) and you'll get a reminder on the specified date (there are options for TO, CC, and BCC, depending on what you need).
